Chronic Disease Self-Management Program (CDSMP)

If you are living with depression, diabetes, cancer, arthritis, high blood pressure, heart disease, etc., then this 6-week program is for you.  This CDSMP workshop is interactive and helps you learn techniques to manage your chronic condition. Workshops are instructed by trained leaders.  Each participant in the workshop receives a copy of the companion book, Living a Healthy Life with Chronic Condition, and an audio relaxation tape.  Classes are highly participative, where mutual support and success build the participant's confidence in their ability to manage their health and maintain active and fulfilling lives.

Diabetes Self-Management Program

If you are living with diabetes or a caregiver of someone who does, this workshop is for you. Eat Healthy, Be Active Workshop Series

Too busy? On a limited budget?  Don't know how to make wise choices about food?  This series can help you make food choices that promote good health, maintain a healthy weight, and prevent chronic disease. These workshops teach individuals how small changes in diet (reducing sugar, salt, fats, and calories) combined with adequate physical activity add up to improved health.

Lunch and Learn

The Speakers Bureau provides speakers on preventive health issues for groups and organizations throughout the community. The purpose of the service is to raise awareness of the importance of preventive health and assist the community on their wellness journey.  The goal is to educate, spark conversation and new ideas.  $50 per topic/session.

Prevent T2

The CDC's Diabetes Prevention Program helps you take control of your health by adopting habits to reduce your changes of developing type 2 diabetes and improve your overall health and well-being.  

The Prevent T2 program provides a supportive environment where participants work together in a small group to learn about healthier eating and increasing their physical activity in order to reduce their risk for developing diabetes.  The program, which is led by a trained Lifestyle Coach in a classroom setting, is delivered over a 12-month period, beginning with 16 weekly sessions followed by monthly maintenance.  In order to qualify for the Diabetes Prevention Program, participants must be at least 18 years old, overweight (BMI>25), and at a high risk for developing type 2 diabetes or have been diagnosed with prediabetes by their physician.
